These Terms of Service govern your use of the Emergency Ambulance Service provided by Family Medical Practice. By using our service, you agree to these terms.
1. Emergency Ambulance Service
Our 24/7 Emergency Ambulance Service provides timely medical assistance and transportation for medical emergencies, staffed by trained professionals.
2. Eligibility
Our Emergency Ambulance Service is available to individuals who require emergency medical assistance within the coverage area of Family Medical Practice. You must provide accurate and complete information when requesting our services.
3. Service Limitations
While we strive to provide prompt and effective emergency medical assistance, there may be circumstances beyond our control that affect our ability to deliver services. These circumstances may include but are not limited to adverse weather conditions, traffic congestion, or other emergencies requiring prioritization of resources.
4. Fees and Charges
As a *9999 member,
you enjoy the following benefits: Unlimited emergency calls and ambulance dispatch services within Ho Chi Minh City (HCMC) during the validity of your coverage plan.
Subscription fee: VND 575,
000 per person per year.
All emergency response services provided by *9999 are covered by your subscription. Fees will only be charged for medical evacuation services outside of HCMC. Additionally,
please note that the facility to which you are transported may charge additional fees for medical attention.
5. Consent to Treatment
By using our Emergency Ambulance Service,
you consent to receive medical treatment as deemed necessary by our medical professionals. This may include but is not limited to emergency medical procedures,
transportation to medical facilities,
and administration of medication.
6. Liability
Family Medical Practice and its affiliates shall not be liable for any damages,
injuries,
or losses arising from the use of our Emergency Ambulance Service,
except to the extent caused by our gross negligence or willful misconduct.
7. Indemnification
You agree to indemnify and hold harmless Family Medical Practice and its affiliates from any claims,
damages,
liabilities,
or expenses arising from your use of our Emergency Ambulance Service.
8. Amendments
We reserve the right to amend these Terms of Service at any time without prior notice. Any changes to these terms will be effective immediately upon posting on our website. Your continued use of our Emergency Ambulance Service after the posting of amended terms constitutes your acceptance of such changes.
